DCA must comply with VA State law and the Department of Social Services requirements. Parents must meet the requirements listed in our Medication Administration Policy.
Medical Forms
The Medication Consent Form must be completed for children who will need medication administered during the school hours. One form must be completed for each medication. Multiple medications cannot be listed on one consent form. Parents MUST complete and sign the form for medication to be administered 10 days or less OR for non-prescription topical medication including sunscreen, diaper ointment, or insect repellent. The Medication Consent Form MUST have Section B filled out and signed be the child's health care provider for Long-Term medications or when dosage directions state "consult a physician"; the parent completes Section A.
